ABC has given a straight-to-series order to an untitled medical drama set in rural West Texas, marking the fourth series in the Grey's Anatomy franchise. The spinoff is co-created by Shonda Rhimes and Grey's Anatomy showrunner Meg Marinis, with Ellen Pompeo also executive producing. The series is described as an edgy drama about a team at a rural West Texas medical centre, the last chance for care before miles of nowhere.

This is the first Grey's spinoff not set on the West Coast or in a big city, and the first that does not spin off a series regular character from the mothership. While the spinoff will focus on new doctors, it may be connected to the original series through characters such as Debbie Allen's Catherine Fox. The premiere is scheduled for midseason 2027.

Marinis, a Houston native, expressed excitement about expanding the Grey's Anatomy universe, noting that the new series will embody the same heart and emotion as the original. The order size has not been determined but is expected to be similar to recent ABC midseason entries, likely around nine episodes.
