Men are increasingly resorting to a controversial practice known as 'penismaxxing' to enhance the appearance and performance of their genitalia. This trend involves a range of methods, from jelqing—using hands or weights to stretch the penis—to 'ballmaxxing', where saline is injected into the testicles for temporary inflation. The latest technique, promoted by influencer Braden Eric Peters (Clavicular), uses lipolysis injections on the mons pubis to break down fat around the genitals.
What Is Penismaxxing and How Does It Work?
The mons pubis is the fatty pad over the pubic bone. When excess fat accumulates, it can cause a buried penis, where a normal-sized organ is hidden by surrounding tissue. Fat-dissolving injections reduce these fat pockets, making the shaft more visible. Dr Babak Ashrafi, GP at Superdrug Online Doctor, explains: 'Reducing excess fat above the penis can create the appearance of increased length. However, this does not actually enlarge the penis itself—it simply changes the surrounding tissue.' He adds that weight loss can improve cardiovascular health and erectile function.
Are These Injections Safe?
Doctors strongly caution against unlicensed use. Dr Shirin Lakhani, an intimate health specialist, notes: 'There are currently no fat-dissolving injections specifically licensed for enhancing penile appearance in the UK.' Products bought online may be counterfeit or contaminated. Dr Ashrafi warns: 'The pubic area contains important blood vessels, nerves, and delicate tissues. Risks include pain, swelling, infection, asymmetry, scarring, and even permanent cosmetic deformity or sexual dysfunction.'
Growing Demand Among Men
Both doctors report rising inquiries about penis enlargement. A 2025 study by Adoreal showed a 30% increase in male aesthetic inquiries, with fat removal being a top treatment. Dr Ashrafi notes: 'Many men are becoming more proactive about body image. Effective weight loss can reduce fat around the lower abdomen and pubic area, improving confidence and penile visibility.'
Medical professionals advise maintaining a healthy weight as the safest approach. Any injectable treatment should only be performed by a qualified healthcare professional after proper assessment.
