Inquest Opens into Death of British Army Officer Killed in Training Incident
An inquest has commenced into the tragic death of a British Army officer who lost his life during a live firing training exercise. Captain Philip Gilbert Muldowney, aged 25, died on January 25 following an incident at the Otterburn Training Area in Northumberland, one of the United Kingdom's largest army training ranges.
Details of the Incident
At Northumberland Coroner's Court on Thursday, senior coroner Andrew Hetherington formally opened the inquest. He stated that Capt Muldowney, affectionately known as Gilbert, was participating in a training exercise at the Otterburn Camp range when the incident occurred.
The coroner revealed that Capt Muldowney suffered a gunshot wound, believed to have entered through his back and exited through his left shoulder. Despite immediate CPR efforts by military medics and the attendance of ambulance crews, he was pronounced dead at the scene.
Ongoing Investigations
Mr Hetherington confirmed that multiple agencies are investigating the incident. Northumbria Police, the Health and Safety Executive, and the Defence Safety Authority are all involved, with the latter conducting a service inquiry. The coroner adjourned the inquest to May 19, 2027, noting this date is provisional and subject to change based on investigation progress.
Background of the Officer
Capt Muldowney, who grew up in Co Roscommon, served as a fire support commander within 129 (Dragon) Battery of the 4th Regiment Royal Artillery. He joined the army on January 5, 2020, and commissioned into the Royal Artillery on December 11, 2020, after completing training at the Royal Military Academy Sandhurst.
His parents attended the hearing via videolink. Following his death, the Ministry of Defence praised Capt Muldowney as an "immensely enthusiastic officer who embraced life with a vigour that inspired those around him."
The inquest highlights ongoing safety concerns in military training exercises, with authorities working to determine the exact circumstances leading to this fatal incident.
