Mum Killed, Daughter Injured After Crossing Lights Failed

Kerry Coombes, a 33-year-old mother, was killed and her eight-year-old daughter critically injured when their car was struck by a train at Houghton level crossing near Preston on June 25, after the crossing lights and barriers failed to activate.

Fatal Incident at Houghton Level Crossing

The crash occurred at around 7:51 am when a Northern service train from Colne to Preston hit the car. Investigators from the Rail Accident Investigation Branch (RAIB) found that the automated barriers, road traffic lights, and audible alarm for pedestrians were not activated, leaving Ms Coombes unaware of the approaching train. The train passed a green signal, permitting it to proceed.

Investigation and Response

The RAIB is investigating the sequence of events and risk management at the crossing. Chris Wright, North West route director for Network Rail, expressed deep sorrow and confirmed full cooperation with the investigation. He stated, "We are deeply sorry that this happened on our railway infrastructure, and for the devastating impact it has had on Kerry Coombes’ family and loved ones." He added that no previous incidents had occurred at this crossing.

Tribute to Kerry Coombes

Ms Coombes, a social worker, was described by her family as having "the biggest heart." They said, "Kerry was a loving daughter, sister, partner and devoted mum who meant everything to us. She had the biggest heart, always put other people before herself." Lancashire County Council also paid tribute, calling her a "highly respected and dedicated" professional.
