According to Philadelphia Mayor Cherelle Parker, the disaster began when a precast roof segment on the top level of the stair tower failed. This initial failure triggered a progressive collapse, causing connected sections across all seven levels to give way in a chain reaction. The structure, owned by the Children's Hospital of Philadelphia, was being built to provide additional parking space for hospital staff.
First responders acted swiftly, evacuating three people from the garage and transporting them to a local hospital. Tragically, one of these individuals was pronounced dead upon arrival, while the other two were treated and released. The identities of the victims have not yet been disclosed to the public.

'We're actually going to deconstruct and delayer this building and that will make it safe for my members to get inside and get down to the lowest levels to ensure that we don't have any survivors down there,' he said. The Occupational Safety and Health Administration (OSHA) has launched an investigation into the collapse, with investigators on the scene since Wednesday. Parker identified a subcontractor, Precast Services Inc, as responsible for installing the precast concrete slab that failed. Additionally, she mentioned that inspection of the prefabricated building segments was the responsibility of Valerie Moody from the consultancy company GAI Construction Monitoring Services. Both Precast Services and GAI have been contacted for comment, though Moody could not be reached.
Aidan Carickhoff, a personal injury lawyer, provided insight into OSHA's focus, stating, 'What OSHA is going to be looking at... is whether there were any issues with either prefabricated materials or the concrete pour of the steps themselves. So they'll be digging back through all of the pour materials and all the construction information to make sure that that part of the job was done correctly.'
