Systemic Failures and Widespread Dismissal of Concerns
The Ockenden report into maternity services at Nottingham University Hospitals NHS trust (NUH) has revealed horrifying systemic failures, including a toxic workplace culture and severe staff shortages. Women consistently reported feeling dismissed, disempowered, or blamed when expressing anxiety or reporting critical symptoms such as reduced foetal movements, severe pain, hypertension, and postnatal deterioration. Their concerns were minimized as maternal anxiety, effectively gaslighting them.
Harrowing Details and Lack of Accountability
The report details harrowing cases: a woman told to labour at home for six days before surgery, a student doctor performing an emergency hysterectomy that accidentally removed a bladder, and a baby's remains disposed of as clinical waste. Nearly half of senior NUH staff refused to speak to the review. Sarah Hawkins, who lost her daughter Harriet, described being made to feel like a “mad grieving parent.” Gary Andrews, whose daughter Wynter died from oxygen deprivation, noted a clinician said, “If we listened to every mother’s concerns, we’d be overrun.” Andrews countered that listening would have saved hundreds of lives.
Deep-Rooted Medical Misogyny
The report found failures were more pronounced for Black, Asian, and ethnic minority women, teenage mothers, and those from deprived backgrounds. This reflects a deeply embedded medical misogyny that combines with racism and classism. Many women share similar stories of being ignored during labour, leading to birth trauma. The Guardian columnist Rhiannon Lucy Cosslett recounted being told she wasn't having contractions and having to demand a doctor.
Calls for Change and Investment
While Martha’s rule—the right to a second opinion—has been extended to maternity units, it still requires patients to advocate for themselves. The government must invest in skilled, valued staff and accountable management to ensure such neglect never happens again. The report underscores the need for a well-funded, humane maternity system as the bedrock of a civilized country.
