The United Kingdom has taken a significant step in obesity treatment as the first GLP-1 weight loss pill, Wegovy (semaglutide), has been officially approved by the Medicines and Healthcare products Regulatory Agency (MHRA). This daily tablet is designed for adults with a body mass index (BMI) of 30 or higher, classifying them as obese, as well as for those who are overweight and have a related health condition such as high blood pressure.

The approval marks a new era in weight management, offering an oral alternative to existing injectable treatments. The medication is intended to be used in conjunction with a reduced-calorie diet and increased physical activity. The dosing schedule starts at 1.5mg, with gradual increases to 4mg, 9mg, and finally 25mg. Patients must remain on each dose for at least one month before moving to the next level.

For those currently receiving private treatment with a weekly 2.4mg Wegovy injection, the MHRA has confirmed that they can transition directly to the 25mg daily tablet. The drug will be available on prescription and accessible through private healthcare channels within weeks, though it has not yet been evaluated for NHS use.

Safety and Effectiveness Data

The MHRA based its decision on clinical trial data where adults took 25mg of semaglutide daily, combined with diet and exercise. After 64 weeks, participants who remained on treatment lost an average of 17% of their body weight, compared to just 3% in the placebo group. When all participants were considered, including those who discontinued treatment, the average weight loss was 14%.

Common side effects reported were gastrointestinal issues such as nausea, diarrhoea, constipation, and vomiting. The medication works by mimicking the hormone GLP-1, which is naturally released after eating, helping to regulate appetite, increase feelings of fullness, and reduce cravings.

Expert and Industry Reactions

Julian Beach, executive director of healthcare quality and access at the MHRA, stated: "Having met the MHRA's rigorous standards of safety, quality and effectiveness, the semaglutide tablet has been approved in the UK for weight loss and weight management. As with all GLP-1 receptor agonists, this is a prescription-only medication."

Sebnem Avsar Tuna, general manager at Novo Nordisk, the manufacturer, commented: "This marks an important milestone for obesity care in the UK. For the first time, people living with obesity have access to a GLP-1 treatment in a daily pill, allowing them the choice and flexibility of oral treatment to support their long-term weight management."

Professor Naveed Sattar from the University of Glasgow welcomed the approval, saying: "The approval of the once-daily oral form of Wegovy (semaglutide) is welcome news for people living with obesity, particularly those who would prefer not to use injections. Expanding the range of effective treatments is important in helping people sustainably reduce caloric intakes within an increasingly obesogenic environment."

James O'Loan, chief executive of Chemist4U, highlighted the potential impact: "The launch of oral weight loss medications is set to be game-changing for UK patients. We've already seen record demand ahead of the expected launch of a Wegovy pill in the coming months, with over 10,000 people joining our waiting list for a consultation. Notably, the majority of people expressing interest have not previously used GLP-1s, indicating that the launch could widen access to millions of people across the country."
