Sam Kerr had given Chelsea the lead in the 77th minute, turning in a header from Sjoeke Nüsken. But Manchester United equalised seven minutes later through substitute Simi Awujo, who smashed home from a free-kick scramble.
With the game locked at 1-1 after 90 minutes, extra time was needed. Girma's decisive strike came in the 97th minute after a corner was half-cleared, with Hannah Hampton's initial save falling to the defender.
Chelsea manager Sonia Bompastor's substitutions proved crucial, with Wieke Kaptein and Sandy Baltimore impressing after coming on. United, who had played in midweek against Atlético Madrid, lacked depth on the bench and could not find an equaliser.
In the day's other match, Arsenal beat Bristol City 3-0 but saw forward Olivia Smith stretchered off after a clash of heads. The Canadian international was fitted with a neck brace before being taken to hospital.
