Bellingham eventually got to his feet following treatment but walked disconsolately around the touchline. Real Madrid coach Alvaro Arbeloa confirmed the player had not been carrying an injury before the match, but said it was too early to make a full assessment.
The injury is a major concern for England manager Thomas Tuchel, who is set to lead the national side in friendlies against Uruguay and Japan next month. England's World Cup campaign begins against Croatia at Dallas Stadium, Arlington, on 17 June.
Bellingham's absence would be a significant blow to England's chances, given his pivotal role in midfield. Further medical evaluation is expected in the coming days to determine the severity of the injury.
