Clarkson's journey with the weight loss injection began following a medical emergency that he described as nearly fatal. While on holiday in the Seychelles in 2025, a failed dive left him struggling, and days later at his Cotswolds home, he experienced chest tightness and numbness. Rushed to hospital, a scan revealed two critically blocked arteries, likely a legacy of his past habit of smoking 40 cigarettes a day. Surgeons inserted two stents to prop open the blood vessels, with a doctor starkly warning he was potentially just "days away" from a fatal event.

"I've had to have completely new outfits, because even my feet have shrunk," he explained. "The shoes I was wearing last year are falling off me as I'm walking around."
