Men are increasingly worried about their fertility, giving rise to a trend dubbed 'spermmaxxing'—a regrettably named practice aimed at improving sperm quality and quantity. While some advice is sensible, such as better sleep and nutrition, other methods like eating raw garlic or dipping testicles in ice water are dubious.
The Anxiety Behind Spermmaxxing
Online anxiety about male fertility is palpable, but is it justified? Experts weigh in on the state of male reproductive health and how to genuinely care for sperm. Research shows a sharp decline in sperm counts: a 2023 meta-analysis found a 51.6% drop in sperm concentration and a 62.3% decline in total sperm count between 1973 and 2018. Factors include obesity, poor sleep, smoking, stress, steroid use, and environmental pollutants like air pollution, says Dr. Sevann Helo, a urologist and fertility expert at the Mayo Clinic.
However, Dr. Justin Dubin, a board-certified urologist, warns that online figures often twist truths to sell products or make outrageous claims, leading to rampant misinformation. 'There is a lot of snake oil now in the sperm business,' notes Lauren Silva Laughlin, CEO of Batch Global, a sperm donor platform that also runs spermaxxing.com. She argues that focusing on men's reproductive health is positive, as fertility has long been unfairly coded as a women's issue.
Signs of Reproductive Health Problems
Semen analysis remains the gold standard for diagnosing fertility issues, says Helo. But for those not ready for testing, sexual function offers a window into testicular health. 'One thing we always look at is your ability to achieve an erection,' says Dubin, noting that spontaneous, nocturnal, and morning erections are good signs. Erectile dysfunction can signal broader health issues, from fertility problems to high blood pressure or depression. Testicular pain or swelling may indicate varicocele, while low testosterone can cause weight gain and fatigue. Inability to conceive after six to 12 months of trying is another red flag.
Common Misconceptions
Misinformation abounds online. A 2023 study by Dubin found that most men's health content on Instagram and TikTok was inaccurate. Key facts: male fertility declines with age, though more slowly than in women. Sperm retention does not improve fertility; in fact, 'stagnant sperm is not good sperm,' says Dubin, who recommends daily sex around ovulation. Testosterone therapy can shut down sperm production, so men with low testosterone should seek alternatives that preserve fertility.
Improving Sperm Health
Lifestyle changes can significantly impact fertility, with a sperm lifecycle of three months offering a window for improvement. Avoid wet heat like hot tubs or saunas, as testicles need to be cooler than the body. Prioritize sleep, as testosterone is produced during REM sleep. A Mediterranean diet rich in whole foods, fruits, and vegetables is beneficial. Resistance training, especially weight lifting, boosts testosterone. Exercise in general helps, but weight training is particularly effective.
