The third leg of the competition took a difficult turn for the pair shortly after they reached their second checkpoint in the town of El Zonte, El Salvador. The illness first manifested in Molly Rainford, who was noticeably absent from a breakfast meeting where Tyler met with fellow competitors Dylan and Jackie Llewellyn.

Molly Rainford described the experience as a "rough 24 hours", speculating on the cause by saying, "I don't know if we ate something dodgy but we got a bit of food poisoning."

When the couple managed to stop for the night at a hotel, Tyler West gave a stark assessment of his condition, stating, "I feel like death. Just grateful we have got a bed to sleep in." He also praised his partner's efforts, adding, "Molly is doing a phenomenal job of looking after me."
