Neill's first notable relationship began on the set of the 1981 horror sequel The Omen III: Final Conflict, where he played Damien Thorn opposite Lisa Harrow. The co-stars soon began dating and welcomed their son, Tim Neill, who later followed his parents into the film industry as a technician and lighting specialist. According to the Sydney Morning Herald, Neill made a rare comment about their 11-year relationship in 2023, stating: “We were together for 11 years. Marriage to Noriko Watanabe and Family Life

After his relationship with Harrow ended, Neill met make-up artist Noriko Watanabe on the set of Dead Calm in 1987, starring alongside Nicole Kidman.
