The coroner noted that Woolley 'took pride in his role and his contribution to policing,' but that the nature of the work 'appeared to affect him.'
His ex-wife, in a statement read to the court, described Woolley as someone who 'doted on' their daughter, who was 'a constant that kept him going.'
Arrest and Suspension
Lancashire Police arrested Woolley on March 27, 2026, on behalf of West Yorkshire Police. He was bailed the following day. The reason for his arrest was not disclosed during the inquest. Following the arrest, Woolley was suspended from work due to the ongoing criminal investigation.
A colleague visited Woolley at home on March 28 for a welfare check. The colleague reported that Woolley was 'visibly upset' and said he 'didn't know how he would cope with everything that was happening.'
Discovery and Toxicology
Woolley's 'lifelong friend' Andrew found him dead after being unable to reach him by phone. The coroner told Andrew that Woolley had spoken to him about 'distressing cases' related to his work and had struggled with mental health. Toxicology tests confirmed that Woolley had taken an excessive amount of medication, which caused his death. He had also written several notes before his death, including one addressed to the coroner.
Formal Complaint and Investigation
After Woolley's death, his ex-wife filed a formal complaint with West Yorkshire Police. An internal investigation reviewed records and body-worn footage from the arresting officers. The coroner stated she was confident 'the correct procedures were followed.'
Coroner's Remarks
Returning a conclusion of suicide, the coroner said: 'In his role as fatalities investigator Derek supported a lot of families in the most tragic circumstances and it is heart-breaking that Derek himself is now the subject of a coroner's inquest.' Woolley, who was born in Leeds, was well-known and respected by many coroners in Lancashire through his work investigating suicides.
