An inmate who scalded his cellmate with a mixture of boiling water and sugar, described as "prison napalm," following a dispute over a vape has died in prison. Danny Clive Smith, 34, was found dead in his cell at HMP Risley in Warrington on November 27, 2025.
Smith's behaviour was said to have been "irrational and unpredictable" during his time in custody. While at HMP Altcourse in Fazakerley, he attacked a fellow inmate in July 2023, leaving the victim with scarring and hearing difficulties. Smith later confessed to guards that he had carried out the assault "because he wouldn't let him use his vape."
The attack led to a court appearance in July 2024, where a judge urged Smith to seek support for his mental health. Smith, originally from Shrewsbury, had dozens of previous convictions for more than 120 offences.
Transfer and Mental Health Struggles
At some point, Smith was moved from HMP Altcourse, a category B prison, to HMP Risley, a category C facility. His mental health continued to deteriorate, with reports of depression, low mood, and suicidal thoughts.
Smith also had a history of accruing debt while at Risley, and his behaviour led to self-harm and suicide attempts. One attempt resulted in him being placed on an Assessment, Care in Custody and Teamwork (ACCT) plan, a specific support mechanism for those at risk of suicide or self-harm.
The ACCT plan was later closed after his mood appeared to improve and no concerns were raised by those who interacted with him. A routine check shortly before 9pm on November 26, 2025, found no issues.
Discovery and Inquest
However, a morning check at 5.27am on November 27 led to the discovery of Smith's body. Despite attempts to resuscitate him, he was confirmed dead by a paramedic at 8.05am.
An inquest was opened in December 2025 and held before Area Coroner Victoria Davies between October 12 and 15. The medical cause of death was listed as asphyxiation by ligature. The inquest concluded that Smith "died as a result of a deliberately self applied ligature."
However, a record of inquest seen by the ECHO determined "his intention at the time of applying the ligature...cannot be ascertained due to the unpredictable nature of Danny's behaviour."
Ongoing Investigation
The Prisons and Probation Ombudsman (PPO), which investigates all fatal incidents in custody, launched an investigation following Smith's death. A spokesperson said: "The Prisons and Probation Ombudsman is conducting an independent investigation into the death of Danny Smith at HMP Risley on November 27, 2025. We offer our condolences to Mr Smith's family and friends."
The PPO investigation continues, and a report into its findings, including whether anything could have been done to prevent Smith's death, is due to be published. A Prison Service spokesperson said: "HMP Risley prisoner Danny Smith died on Thursday, November 27. As with all deaths in custody, the Prisons and Probation Ombudsman will investigate."
