An inquest has been opened into the death of a 96-year-old man who was found with a puncture wound in a blood-soaked room at his care home. Glyn Hughes died at Plas Hafan residential care home on Ffordd Dewi Sant in Nefyn, Gwynedd, on October 25.
Welfare check and discovery
Sarah Riley, assistant coroner for north west Wales, said she had been informed that during the night of October 24 to October 25, Mr Hughes had been under the care of two members of staff. A welfare check was carried out at 3.15am, during which a carer noticed he was in bed, but blood was visible on the bedroom floor.
"She called for assistance and the emergency services requested an ambulance," said Ms Riley. A paramedic attended the scene and confirmed Mr Hughes had died at 3.44am.
Police investigation and post-mortem
Police were subsequently called, with officers finding Mr Hughes lying on the floor. He had sustained a cut to his right forearm, and blood staining was present on both the bedding and the wall, Ms Riley added.
Home Office pathologist Dr Brian Rodgers carried out a post-mortem examination, providing a provisional cause of death as haemorrhage resulting from a puncture wound to Mr Hughes' right arm. It was noted that he had been receiving anti-coagulant treatment at the time.
Inquest adjourned
The coroner adjourned the inquest to a later date, pending further investigations.
