Tijjani Reijnders had given City a first-half lead, but the home side's wastefulness proved costly as Fernandez bundled home from close range.
The match was the first for Chelsea under interim manager Calum McFarlane, who took charge after Enzo Maresca was sacked on New Year's Day. McFarlane made five changes from the side that drew 2-2 with Bournemouth, including Filip Jorgensen replacing Robert Sanchez in goal and Moises Caicedo serving a one-match ban.
City dominated the first half and took a deserved lead through Reijnders, who scored for the third consecutive Premier League start with a powerful strike past Jorgensen. However, the visitors grew into the game after the break, and City's inability to add a second goal proved costly.
Pep Guardiola was forced into an early substitution when Josko Gvardiol injured himself attempting to dispossess Malo Gusto, with Abdukodir Khusanov replacing him. Rodri impressed on his first Premier League start in three months, creating four chances and winning seven duels, but his teammates were profligate in front of goal.
In stoppage time, Gusto raced past Nico O'Reilly and crossed low for Fernandez, whose initial effort was saved by Gianluigi Donnarumma before he stabbed home the rebound. The result leaves City vulnerable in the title race, while Arsenal will view the draw as a major boost to their own ambitions.
