Former child actor Blake Garrett, best known for playing Plug in the 2006 family comedy How To Eat Fried Worms, died from an accidental fentanyl overdose, according to a report from the Oklahoma Office of the Chief Medical Examiner.
The report, cited by TMZ, lists Garrett's cause of death as acute fentanyl toxicity and rules the manner accidental. He died on February 8 in Tulsa, Oklahoma, at the age of 33, at a sober living house.
Garrett's mother previously told TMZ that her son had visited an emergency room a week before his death after experiencing intense pain and was diagnosed with shingles. She expressed concern that he may have self-medicated after three years of sobriety, and believed his death was a tragic accident.
Born in Austin, Texas, on September 14, 1992, Garrett began performing at age eight in local productions, taking lead roles in Aladdin and his Magical Lamp and Peanuts: A Charlie Brown Tribute. His other theatre credits included The Wizard of Oz, Annie and Grease.
Garrett shared a Young Artist Award in 2007 for Best Young Ensemble in a Feature Film for How To Eat Fried Worms, which was written and directed by Bob Dolman and based on Thomas Rockwell's 1973 children's book. He also appeared in Barney's Colorful World International Tour from 2003 to 2004, playing the role of Mike.
